Telehealth Consent & Patient Communication Policy

ChristCare Wellness Center offers telehealth services to improve access to healthcare while maintaining high standards of patient care, privacy, and security.

What is Telehealth?

Telehealth allows patients to receive healthcare services through secure electronic communications, including video conferencing and other approved technologies.

Patient Acknowledgements

By participating in telehealth services, patients acknowledge that:

  • Telehealth may not be appropriate for all medical conditions
  • Technical difficulties may interrupt communication
  • Certain examinations may be limited when conducted remotely
  • Alternative treatment options may be available, including in-person visits
Privacy, Security & Emergencies

ChristCare Wellness Center utilizes reasonable safeguards and secure technologies in accordance with HIPAA. Patients are encouraged to participate from a private location using a secure internet connection.

Telehealth services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or proceed to the nearest emergency room immediately.

Patient Responsibilities & Communications

Patients agree to provide accurate health information, participate from a safe and private location, notify the provider of technical difficulties, and provide updated contact information.

ChristCare Wellness Center may communicate through secure patient portals, telephone calls, text messages, or email as permitted by law and patient consent. Patients should not use email or website contact forms to communicate urgent medical concerns.


Appointment Cancellation & No-Show Policy

At ChristCare Wellness Center, we strive to provide timely, high-quality care to all patients. Missed appointments and late cancellations limit our ability to serve other patients in need of care.

Cancellation Policy

If you are unable to attend your scheduled appointment, we ask that you provide at least 24 hours' notice whenever possible. Appointments may be cancelled by:

  • Calling our office during business hours
  • Using the patient portal (if available)
  • Following any appointment cancellation instructions provided at scheduling
No-Show & Late Arrival

A "no-show" occurs when a patient fails to attend a scheduled appointment without prior notice, or arrives too late to be accommodated within the provider's schedule.

Patients arriving more than 15 minutes late may be asked to reschedule. Multiple missed appointments may affect a patient's ability to schedule future appointments and may require review by practice management.

Missed Appointment Fees

ChristCare Wellness Center reserves the right to assess a missed appointment fee for no-shows, late cancellations, and repeated missed appointments. Applicable fees will be communicated to patients in accordance with applicable laws, insurance requirements, and contractual obligations.

We understand that emergencies and unexpected events occur. Special circumstances will be reviewed on a case-by-case basis.
